We are accepting new patients including "out of area".
We are happy to welcome new patients to register with the practice. It is very easy to register with the practice wherever you live. We can register a patient as urgent if necessary and will process the registration as quickly as possible.
Most of our patients live in Kingston and the surrounding area, however we welcome patients from a wider are if they wish to register with the practice. Patients living outside our catchment area can access all services apart from home visits and some community services.
Please complete the new patient registration online form by clicking on the link below and our team will process your application within 2 working days. If you need to be seen urgently before your registration is complete, please get in touch.
Photo ID is only required if you are registering in this country for the first time, and you will need to bring this with you on your first visit to the practice. Otherwise, no ID is necessary, except for registration to our 'historical' online services.
Before any medication can be issued you must be seen by one of the doctors or nurse practitioner.
We can arrange Interpreter's services as well as a Chaperone if requested.
For more information on registration for online services please go to the section "Online Access".
Please ensure you have read the information in these downloads before completing the online registration form below.

NHS Organ Donation Registration
The law has changed so that you now need to "opt out" if you do not want to become a Donor. It is important that you talk to your family about your organ donation decision as they will be asked to support your decision. If you are undecided or do not want to become an organ donor please contact the NHS Organ Donations website at www.organdonation.nhs.uk or tel: 0300 123 23 23.
