When you register as a new patient it can take some months before we receive your old medical records.
We ask that patients registering with the practice who are over 40 please make an appointment for a new patient registration check.
This is so that we may obtain details of your past medical history, assess any current needs and perform a simple health check as soon as possible, after registration.
Proof of Identity: Passport, driving licence with photograph
Proof of Address: Utility bill, letter from landlord, letter from employer.
We also require:
A copy of marriage certificate if just married, legal document if just change of name.
TB Status: Where Applicable.
For your convenience you may print off a registration form and registration documents.You can then fill these out and bring them in with you on your first visit to the practice.
Also if you want to 'opt out' of the Summary Care Record Scheme please download a copy of the 'opt out' form here. If you wish to be part of this scheme then you need do nothing and you will automatically be entered on. For more information about other initiatives you may wish to opt out from please go to the section "Sharing your records".
