New Patient Registration - Adults


CORONAVIRUS PATIENT UPDATE

 DUE TO THE CORONAVIRUS OUTBREAK WE WILL BE MAINLY MAKING APPOINTMENTS OVER THE TELEPHONE AND NOT IN PERSON AT THE FRONT DESK UNTIL FURTHER NOTICE. PLEASE TELEPHONE US ON 020 8546 3136 TO ARRANGE AN APPOINTMENT.

ALTERNATIVELY YOU CAN USE THE DOCTORLINK SERVICE.

IF YOU ARE BOOKED FOR A FACE TO FACE APPOINTMENT WITH A CLINICIAN PLEASE BE AWARE OF THE FOLLOWING:

  • When you arrive at the practice please ring the buzzer and wait to be escorted into the building by a Clinician who will check your temperature at the door.   (All our clinicians will be wearing PPE).
  • Your must wear a face covering / mask
  • Please attend alone if possible, unless you are in the vulnerable group or bringing a child

PATIENTS AND STAFF

When you register as a new patient it can take some time before we receive your old medical records.

This is so that we may obtain details of your past medical history, assess any current needs and perform a simple health check as soon as possible, after registration.

NEW PATIENT REGISTRATION - ADULTS

You can now register on line by completing the forms below and submitting your details to the practice electronically.   Photo ID in only required if you are registering in his country for the first time, and you will need to bring this with you on your first visit to the practice.    Otherwise no ID is necessary, except for registration to our online services.

Before any medication can be issued you must be seen by one of the doctors or nurse practitioner.

NHS ORGAN DONATION REGISTRATION

The law has changed so that you now need to "OPT OUT"  if you do not want to become a Donor.   It is important that you talk to your family about your organ donation decision as they will be asked to support your decision.  If you are undecided or do not want to become an organ donor please contact the NHS Organ Donations website at www.organdonation.nhs.uk or tel: 0300 123 23 23.
