Throughout the COVID-19 pandemic we have offered appointments using new ways of working and new technology, i.e. (video and Doctorlink)

We would really appreciate your feedback and any thoughts.  Please complete the survey online below.

Thank you for being patient with us as we are going through a lot of changes.

We will continue offering telephone / video consultations as a default appointment and you will be able to pre-book these in advance (at least 4 weeks).   There will be certain appointments, and when essential, that will be offered as face to face appointments.

Information entered into survey forms is used only for the purposes of processing your survey information and is stored and accessed securely by designated staff.

Issues raised in comments may be discussed between relevant members of staff. The information is used for quality monitoring purposes, in line with the expectations of those submitting the feedback.

Any personal information transmitted via survey forms may be anonymised when this is required to ensure compliance with General Data Protection Regulation.

This information is not shared with any external third party organisations.

This information is kept for as long as the site is active or it is deleted by a site administrator.
